Our client is looking for a senior project manager to deliver a number of strategy commissioning priorities across Adult Social Care.

Who we're looking for
The ideal Commissioning Project Manager will fulfil the job Purpose is as follows:-

• Review how domiciliary care is currently provided and develop a preferred option which sets out how to deliver Domiciliary Care across all Adult Social Care client groups in the future. This will need to support personalization and an outcome based approach.

• Implement a sustainable model for Adult Placement across all care groups.

• Modernise services which formed part of hospital resettlement for Learning Disability services and develop capacity to meet the requirement of current campus services. This will need to consider the housing and support components as well as the funding regimes required to support the delivery.

• Implement the outcome of a review of Emergency Duty Team and work with the PCT to ensure that social care and health emergency out of hours services are delivered in an integrated way.
