Job Description

  • Provide patient care in their own homes following a pre-agreed program of care and care plan.
  • Conduct accurate, holistic assessments of patient's needs in accordance with the approved model of care.
  • Safely administer a range of therapeutic interventions relevant to the NHS project and training, as per agreed clinical pathways within the scope of practice.
  • Help patients with their daily activities and support the multidisciplinary team's goals of enhancing their level of independence.
  • Fulfil roles and responsibilities according to the NMC Code of Practice and HAH policy, procedures, and protocols.
  • Take responsibility for personal health, safety, and welfare, adhering to the Health and Safety regulations and reporting any accidents or incidents using the relevant documentation.
  • Fully comprehend and understand the duties and responsibilities of safeguarding all patients, both adults and children, through relevant training provided
  • Recognise the types and signs of abuse and neglect related to vulnerable adults and children, and inform the relevant line manager of any concerns.

The Successful Applicant

  • Registered General Nurse with Part 1 NMC Register (or RSCN where appropriate).
  • Experience in an acute care setting after registration, including making independent clinical decisions and supervising others work.
  • Strong knowledge of relevant therapies and drugs, including NHS protocols for managing patients with treated conditions.
  • Attentive to detail and adheres to rules, protocols, and quality standards.
  • Excellent communication skills, including active listening, sensitive communication, creating a comfortable environment, and presenting technical information in an understandable way.
  • Adaptable to change; flexible enough to handle evolving service demands.
