Job Description

  • Provide practical and emotional support to vulnerable adults, including welfare checks and check‑in calls.
  • Deliver flexible, reactive support to individuals following hospital discharge.
  • Respond to referrals from clinicians and community health professionals.
  • Assess service users' needs and follow an agreed support plan.
  • Assist with essential tasks such as attending appointments, shopping, and collecting prescriptions.
  • Help prevent hospital readmission by offering ongoing tailored support.

  • Professional, confidential, and able to handle sensitive information appropriately.
  • Comfortable working independently and travelling across the local area in a service vehicle.
  • Knowledge of how NHS and social care services operate and how to signpost effectively.



Essential Requirements

  • Full UK Manual Driving Licence (mandatory for driving service vehicles).
  • Valid Enhanced DBS Certificate
  • Ability to work a flexible schedule within weekday hours.
  • Right to work in the UK.
